Upgrade to the latest version of the QLM WooCommerce plugin.
In your QLM Settings in WordPress, enable logging as per the screenshot below.
Reproduce the issue you are trying to troubleshoot.
In your WP site, locate the log file associated with the order as per the 2nd screenshot below.
Send us a copy of the logs.
